When to fill out Form HW027
First-time registration
You are registering a practitioner or practice for Medicare online claiming (such as Bulk Bill, DVA, or ECLIPSE) for the first time.
New practice location / Relocation
You are setting up a primary/first location or moving your practice to a new address.
New provider/doctor
A new practitioner is joining the practice and needs their individual provider number linked to the online claiming setup.
Changing practice management software (PMS)
You are switching software integrations that require a new provider agreement.
HW029 — Provider Registration for Electronic Funds Transfer Payments
For registering or updating the bank account details used to receive Medicare payments by Electronic Funds Transfer (EFT).

When to fill out Form HW029
Multiple locations / Linking provider numbers
You are linking a provider number to additional practice locations (typically up to three additional locations per form) or registering a single doctor who has multiple provider numbers across locations.
Adding/updating EFT bank details
You need to add or change the bank account details linked to specific provider numbers.
Alternatively, instead of using the paper HW029 form to update bank details, you can update them online via HPOS (Health Professional Online Services) using a PRODA account.
